The FDA cleared aprevo lumbar implants via the 510(k) pathway in December 2020 and cervical implants in November 2024, and as of December 31, 2025 the company had 253 surgeon users, up from 152 a year earlier, out of an estimated 4,000 addressable U.S. spine surgeons.

Carlsmed earns revenue by selling aprevo surgical kits — personalized interbody implants and single-use instruments matched to each patient's imaging-derived anatomy — with lumbar procedures reimbursed by Medicare, Medicare Advantage, and commercial payors under premium-paying MS-DRG codes relative to stock-implant procedures. In August 2025, CMS finalized new X-codes and a New Technology Add-on Payment of up to $21,125 per qualifying inpatient cervical spine fusion procedure, effective October 1, 2025, ahead of Carlsmed's December 2025 commercial launch of its cervical platform and corra Cervical Plating System. Because each implant is manufactured to order for an individual patient's anatomy, Carlsmed relies on a limited number of contract manufacturing organizations (CMOs) for the manufacture, treatment, sterilization, packaging, and distribution of its products, an on-demand, customized model the company describes as expensive and time-consuming to manage against variable surgical demand.

Carlsmed's structural exposure is concentrated in its manufacturing supply chain rather than in a single payor or geography: because each aprevo implant is custom-built to a specific patient's pathology, the company cannot simply hold finished-goods inventory as a buffer, and it says reliance on a limited number of CMOs for manufacture, sterilization, packaging, and distribution increases the risk that it will not have sufficient product quantities, or at an acceptable cost, if a CMO cannot meet demand on a timely basis.
